Meridian's diagnostic arm was spending two full-time coordinators on confirmation calls and still losing a fifth of its booked slots. Every empty slot was a scanner sitting idle and a patient waiting a month longer than they should.

The rehearsal run that failed 188 times
Northwind's first submitted rule set passed every design review and failed its very first rehearsal run. One adversarial persona — a caller claiming to be the borrower's spouse — extracted a balance readout in 188 of 10,000 simulated calls, because the identity check ran after the disclosure instead of before it.

Kartelo was losing 9% of cash-on-delivery shipments to returns-to-origin — pure margin surrendered to a courier truck. The two causes were bad addresses and unreachable buyers, and both were fixable before dispatch rather than after.

Vantek was buying overtime every night to cover a four-hour spike and replacing a fifth of its tier-1 agents each quarter. The work causing both problems was repetitive, scripted, and exactly the work a voice agent should have been doing.
